Priory's purpose is “To Make a Real and Lasting Difference for everyone we support”. Working alongside our Healthcare Team to help support transitions for individuals from hospital into community based settings.

This role is offered on a hybrid remote basis, the successful applicant must have a Driving Licence and access to a car.

We have an exciting opportunity for a Assessment & Transition Director for Complex Autism to join the team and support our growing business. Reporting to the Service Development Director - Adult Care, the Assessment & Transition Director will be responsible for:

  • Reporting to the Adult Care SDD, the Assessment & Transition Director for Complex Autism is accountable for supporting sites to assess and transition service users with complex autism into services to prevent placement breakdown.   
  • To have an overview of assessments; ensuring thorough assessments take place and detailed accordingly
  • The role will work collaboratively with the, Managing Directors, Operations Directors and Service Development to appropriately place, ensure site is ready and see them through till settlement. 
  • The role will work cross divisionally with Healthcare supporting pathways to community placements from Healthcare hospitals
  • To lead on BTP within Healthcare and ensure a community plan is in place

Job Advert Location Description

Assessment and Transition Director duties include but are not limited to:

  • Accountable for proactively driving improvements in performance and quality, ensuring there is consistency of delivery across all Autism Services to ensure quality indicators, care standards and regulatory requirements are met. This will require collaborative working with the Quality Team.
  • Strategically identify quality issues and operationally set objectives with Autism services and key stakeholders. Develop measures and ensure the tracking and monitoring of their progress, continually driving and positioning quality to be embedded in service delivery.
  • Accountable for working collaboratively with the Operations Director to ensure the workforce in Autism services are competent to care for people with autism, creating a caring environment that yields positive service user outcomes and experience.
