Job Advert Role Description

Avesbury House is a male only low secure forensic 24 bed service.

It supports the reintegration of service users back into the community.

Collaborative approach within MDT which supportive and tailored to the individual to allow a successful discharge pathway with the focus being on relapse prevention, independent living, social inclusion (including vocational engagement) and risk reduction.

Responsibilities of the post include:

• Delivering the very best care and treatment to patients by managing the assessment, implementation and evaluation of individual patient care plans while promoting a professional working environment to ensure a high standard of patient care.

• Working with our patients to ensure care and treatment plans are being followed and patients are working towards their recovery goals.

• Ensure effective communication of any concerns relating to patient care.

• Working as part of the nursing team to maintain patient safety by regularly reviewing risks during the shift.

• Work as a Keyworker to patients-this includes working closely with them to complete risk assessments, care plans, reports for tribunals and CPAs and supporting them in their meetings with the rest of the MDT.

• Administering medication appropriately and in accordance with Priory policy and NMC guidelines.

• Undertaking a supervisory role to junior staff

• Completing mentorship training (if you don’t already have a qualification) and supporting students during placements.
