We also offer a range of residential and nursing care settings for older people with varying levels of need.

Job Advert Location Description

Dalton Court is a purpose built care home providing a friendly and companionable place to live for older people requiring general or specialist dementia nursing care. Located in the heart of the Lake District, the home offers a supportive environment with individualised bedrooms and comfortable lounge areas. 

As a Care Home Advanced Practitioner your key responsibilities & accountabilities will include the following:

  • Be compliant with Priory Adult Care Policies, Procedures, National care standards, Regulations, codes of practice, and legislative requirements to help provide a safe service to Residents.
  • Promote and support accurate reporting and recording by colleagues as appropriate. Ensure that all incidents/accidents are reported in accordance with company and Regulatory requirements to help maintain the safety and welfare of Residents at all times.
  • Act as a positive role model for Colleagues in terms of behaviour and attitudes, and as required, participate in recruitment processes and the learning and development of others.
  • Closely monitor Residents who may be confused/have a diagnosis of dementia and may demonstrate behaviour that challenges. Co-ordinate the care they require to safeguard their wellbeing and the safety of colleagues supporting them.
  • Comply with the company’s medicines management arrangements ensuring medicines are appropriately received, stored and administered. Concerns about a Residents health or medicine errors will be reported promptly.
  • In conjunction with colleagues liaise with GP’s and other health and social care professionals to ensure that all aspects of the Residents’ health and social care needs are met.
  • Ensure full privacy and dignity is maintained for the dying and the bereaved, in line with best practice and the Company’s policies and procedures.
  • Assist the Home Manager for the delivery of clinical services and nursing practice in accordance with regulatory requirements and take responsibility within their absence.
  • Assist in the delivery of care for Residents who are dying or who have a progressive illness. Lead the care and support of a Resident and those close to them following the death of a Resident, respecting choices and decisions they made about their wishes when they die and any cultural/ethnic arrangements necessary.

What we are looking for:

  • Completion of the Care Home Advanced Practitioner (CHAPs) Programme
  • Evidence of CPD and a positive approach to continuous training and development.
  • Knowledge and understanding of person centred care planning process and the relevant legislative and Regulatory requirements.
