Job Advert Role Description

The Priory Hospital Middleton St George is located close to Darlington, County Durham and provides individualised treatment and care for people with mental health needs of adult working age and who present with complex and challenging behaviours, in a safe and supportive environment. Our comprehensive integrated services offer complete care pathways allowing service users to transfer effortlessly between the most appropriate care, meeting individual needs enabling recovery and re-engagement within the community and maximising life opportunities.

Our multi-disciplinary teams provide care for up to 101 male and female service users, working collaboratively with their commissioners, across our 7 wards and our nearby community house.  We deliver Female Acute, Mixed Acute, Male and Female PICU services, and a recovery and rehabilitation environment which includes a specialist female Personality Disorder service.  Our focus is on the delivery of excellent care, empowering service users to progress towards the least restrictive setting and towards independence.  Our vision is to place our Service Users at the heart of our service provision, working closely with them and their carers to support their recovery and rehabilitation.   

The role of the Social Worker is to participate as an active member of the Multidisciplinary Team. The Social Worker is allocated to work across site and participates in ward rounds, risk assessments, care planning, discharge planning, decision-making and team meetings. The Social Worker’s responsibilities include:
 To engage effectively with service users to assist with each person’s rehabilitation and recovery;
 To provide a link with service users’ locality services and the Multi-Disciplinary Team;
 To provide a link with service users’ social supports (carers, family members, friends) and the Multi-Disciplinary Team;
 To act as Carer Champions for the hospital and promote carer engagement in service users’ treatment as appropriate; 
 To support and encourage visits by making sure visits are supportive and beneficial to service users;
 To ensure child visits are in the best interest of the child and properly risk assessed prior to a visit being approved; 
 To identify, assess and contribute to the management of risk;
 To act as a lead in the multi-agency contact relating to matters of Safeguarding At Risk Adults, Children and Young People, acting as Appropriate Adult for police interviews, and supporting MAPPA/MARAC needs; and
 To identify and challenge discriminatory structures and practices.
