Job Advert Role Description

Strathmore College are seeking a Referral & Transition Co-ordinator to join their friendly team.  The successful candidate will work proactively within a person centred culture to enable young people to develop confidence, skills and coping strategies that prepares them for opportunities, responsibilities and experiences of adulthood and enables them to achieve aspirational outcomes.  The post holder will work as part of a committed and highly specialised multi–disciplinary team. Required to exercise direction in providing and ensuring a high quality, safe and effective assessment and transition service.  

Duties include:

  • Developing links with providers to build relationships in order to improve the transition of learners
  • Working within a multi-disciplinary team to formulate all pre entry assessment, to include the coordination and identification of areas of risk for all proposed learners
  • Liaising with schools, providers, parents and stakeholders to coordinate the assessment and transition process
  • As required, attending initial transfer reviews ensuring the outcomes are recorded and effectively communicated to the Wellbeing Lead
  • Producing proposed learner profiles in a positive person centred way that highlights the strengths and areas of support
  • Ensuring that all relevant staff are inducted into the needs of the learner prior to assessment and placement
  • Contributing to the assessment by working alongside learners, ensuring needs are met and learners are well prepared for the assessment
  • As required, plan assessment activities that will be motivational to the learner and create a positive environment in which to assess
  • Producing pre-placement assessment reports identifying the learners support needs, outline of initial programme to inform decision making
  • On receipt of the Education Health and Care plan ensure that it is highly reflective of need, accurate and has learning outcomes identified
  • Become a designated safeguarding lead and contribute ensuring that Safeguarding is a high priority and policy and procedures are followed at all times.

The successful candidate will have a good understanding of the needs of learners’ who have a diagnosis of Autistic Spectrum Disorder, mental health and associated learning difficulties and will continuously develop skills and knowledge to meet the growing complexity of learners needs.

Strathmore College is a residential and day college in Stoke on Trent.  The college provides for learners aged 16-25 years who have learning difficulties, emotional, social and behavioural difficulties/disability. This can include young people with trauma, attachment and challenging behaviours. The cohort of learners’ needs include mild to severe learning difficulty/disability, Downs Syndrome, Autistic Spectrum Conditions, speech and language difficulties coupled with other health related, mental or physical difficulties.  The learner’s needs are increasingly more complex, with a number of learners who were NEET and those who were finding engaging in education and learning challenging. The College uses meaningful, practical learning activities to engage and motivate learners.
