Job Advert Role Description

Kings Dale Close Supported Living Service 

Located in the village of Walton-le-Dale in Preston, Priory Adult Care have worked in partnership with Pathways Housing to develop two supported living apartments. 
The new step down service is designed to further support people to live as independently as possible within their own tenancy, and in the local community.
 Kingsdale Close will provide supported living self-contained apartments, offering single tenancy and communal space. 
These two apartments are on the ground floor and first floor of the property, and are ideal for people moving on from hospital settings or residential environments, who require high support needs. The facilities and equipment available will include:
• 2 self-contained apartments, each with their own kitchen, bathroom and seating area
• A spacious communal area
• A garden area 
• Staff availability 24 hours per day 


In addition to the Job Description - We would like our Support Workers to embrace the Positive Behaviour Support approach, enabling the people we support to improve their quality of life through skill building. We use the Person Centred Active Support model to enable individuals to achieve skills and develop their confidence. 
We would like people with a "Can Do" attitude and are solution focussed. We will provide support to our staff however we do expect a degree of autonomy and responsibility. Communication is key to ensure we create a Team Based approach to help us to achieve our goals
