Priory Adult Care- Supporting You In London and Thames Valley is a service that provides accommodation and support for adults with learning disabilities and/or mental health support needs. Our team provides community accommodation and outreach support throughout the London and Thames Valley area.

SYI in London and Thames Valley provides supported living tenancy's and outreach support to vulnerable adults in the community with learning disabilities, mental health, challenging behaviour and autism. We have block of flats and shared houses. 

Frays is a house which supports 4 individuals who live with their own tenancies. They are very active in the community and enjoy living independently with support.

Job Advert Role Description

As a Locality Coordinator you will deputise for the Locality Manager in the operation of the service including service user care and personnel.

The Locality Coordinator is required to assist and support service users to live full and independent lives meeting a range of personal care, social and emotional care needs.  The role requires directly supporting individuals in their own homes with daily living skills, household duties, managing finances, medication, shopping, personal hygiene, health appointments, accessing the local community and social engagements or employment, and supervising support workers carrying out these duties.

The Locality Coordinator responsibilities include:
 

  • Responsibility for the day to day management of the service during the absence of Locality Manager, taking responsibility for the day to day management of the project
  • Oversee, implement, review and work alongside My Plans, Guidelines, and Risk Assessments 
  • Oversee and complete daily handover records; oversee and audit the ordering and administration of medications; oversee and complete any health/behaviour monitoring charts; oversee and complete Monthly Service User Reports
  • Oversee and complete monthly My Meetings 
  • Ensure individual’s files are kept up to date and oversee monthly Quality checks for your allocated sites
  • Oversee, co-ordinate and attend reviews and support the recruitment of staff and applicant interviews
  • Oversee petty cash and CFA transactions, ensuring a clear audit trail and records are kept and audited
  • Ensure and maintain good standards of hygiene and food safety when supporting individuals 
  • Write, and complete the rota covering the contracted hours for each support package. Cover, record and report sickness and changes to the rota; ensure benefit applications are completed correctly and in a timely fashion as required by service users
