Job Advert Role Description

Ty Cwm Rhondda is an independent low secure hospital located in Ystrad, Rhondda Valleys. The hospital is a 20-bedded facility offering high quality assessment, treatment and rehabilitation services for adult males with complex mental health needs and serious mental illnesses including treatment-resistant presentations and challenging behaviour. Ty Cwm Rhondda has a 3 Q quality rating following their recent inspection why the NHS Quality Inspection Team.

We seek to work with individuals to stabilise their mental health and behaviours with the aim to maximise independence and promote re-integration into the community at the earliest opportunity.

We offer services to those who are liable to be detained under the Mental Health Act. We work with individuals whose mental health and/or behavioural difficulties compromise their psychological wellbeing and who are considered inappropriate for open, acute inpatient services. We support those who may be stepping down from

higher levels of security or from the criminal justice system, including prison, court or police custody.

The multidisciplinary team comprises of Psychiatry, Psychology, Social Work department, Occupational Therapy department, Practice Nurse and a compliment of Mental Health and Learning Disability trained nursing team.

The site facilitates and runs recovery college courses, reducing restrictive practice and physical health initiatives. 

Ty Cwm Rhondda hosts a number of friends and family events throughout the year including quarterly meet the team events. Ty Cwm Rhondda also has a number of staff events throughout the year focussing on wellbeing.

Our Social Worker role involves:

Delivering a set of agreed service -level standards within a personalised Recovery Model of intervention.

Undertaking to complete comprehensive social histories and risk assessments.

Coordinating discharge-planning alongside the multidisciplinary team.

Providing consultancy and overseeing in safeguarding vulnerable adults and children as the designated safeguarding lead.

Writing MHRT reports, attending hearings and providing oral evidence.


Writing and collaboratively producing care plans alongside service users and attending professional meetings.

Undertaking carer assessments and carer support.

Working collaboratively within a multi-disciplinary team

Being Committed to anti discriminatory and anti-oppressive practice, and to empowering service-users to the fullest extent

Coordinating financial assessments and the provision of patient benefits and monies where appropriate

Liaison with external statutory agencies including police, local authorities, MAPPA and probation.
