Assessment Transition Director

With community-based residential care settings, as well as a network of supported living services, we support adults over the age of 18 with autism, dementia, learning disabilities, mental health conditions, Prader-Willi Syndrome (PWS) and behaviours that challenge. We also offer a range of residential and nursing care settings for older people with varying levels of need.

Priory Adult Care are recruiting for an Assessment Transition Director that will be responsible for the following:

  • Gather key information on complex assessments
  • Liaise with commissioners to understand requirements and ensure thorough understanding & assessment of Service User needs
  • Ensure the site is admission ready from a competency and environmental point of view
  • Support the site through the initial 8 weeks of admission, which can often be a turbulent time for somebody with Autism
  • Support & up-skill staff, increase their competence and provide reassurance to ensure placements do not break down
  • Assess and improve competence of staff across the business more widely around autism & act as the Divisional Subject Matter Expert in relation to complex autism

This role will cover aspects of service development, operational settling of complex admissions, key liaison with site and commissioner and also as a staff retention aid.

The successful candidate will:

  • Have experience with complex admissions under transforming care
  • Positive behaviour or practitioner experience at a senior level.
  • Background in Autism, Mental Health, complex behaviours.
  • Diploma level 5 or higher
  • Understand the concepts of creating autism friendly environments to meet sensory needs.
  • Experience in supporting people with complex communication needs, I.e. Makaton/BSL so to ensure service user can be included where possible to ensure person centred.
  • Good relationships and communication with external professionals.
  • This is a national role, so travel will be required with overnight stays where needed. Occasional unsociable hours to support placement when required.
