With community-based residential care settings, as well as a network of supported living services, we support adults over the age of 18 with autism, dementia, learning disabilities, mental health conditions, Prader-Willi Syndrome (PWS) and behaviours that challenge. #LoveYourJob! Supporting You in Worcestershire and Gloucestershire, is looking for a Locality Coordinator to support individuals who live in their own shared house, to join an award winning team, who recently were awarded the LaingBuisson Social Care Award for outstanding Supported Living.

You will need to be flexible, as the position will include shift work and sleep-ins, have a great sense of humour, be able to listen, patient, and most of all be passionate about person centred support.

Locality Coordinators are crucial to the provision of this support ensuring that the highest quality of support is given to our clients.

Diploma Level 3 in Health & Social Care is essential.

To support the Registered Manager in overseeing the day to day management of the identified service(s) to provide the highest standards of support to the people who use our service in line with company and regulatory standards.  This role will generally be responsible for one or 2 projects. Although this can vary dependant upon care hours/ geographical location.

Assist the Locality Manager and support staff to ensure that the people who use our service have any personal needs or requirements met; including personal care, participation in activities and support with daily living tasks

-Health reviews

-Monthly review meetings

Ensure that the service provided meets the Reach standards requirements and Supported Living values and principles

-Monthly audits/ medication/ finances/ H&S

Complete daily administration including updating person centred support plans, updating charts and completing accurate daily notes, which are reviewed regularly with service users/their representative/appropriate multi disciplinary team.

Maintains professional relationships whilst developing a good rapport with the people who use our service, their family, colleagues, visitors and other stakeholders.

Through own professional behaviour, including adhering to policies and procedures, ensure the maintenance of safety, security and confidentiality standards, for themselves, colleagues and service users
