With community-based residential care settings, as well as a network of supported living services, we support adults over the age of 18 with autism, dementia, learning disabilities, mental health conditions, Prader-Willi Syndrome (PWS) and behaviours that challenge. We also offer a range of residential and nursing care settings for older people with varying levels of need.

Supporting You in The South East (SYI New Romney) is dedicated to providing individually tailored packages of high quality, person centred support to individuals with learning disabilities, associated conditions and mental health issues to develop the skills they need to live as independently as possible. Support can be provided in accommodation-based settings or via our outreach services.

We provide support to people who are funded directly by the Local Authority, have a direct payment or individual budget and some people buy in support privately.

 

Job Advert Role Description

Locality Coordinators are required to support and enable service users to live full and independent lives meeting a range of personal care, social and emotional care needs. Locality Coordinators are required to support service users in their own homes with daily living skills, household duties, managing finances, medication, shopping, personal hygiene, health appointments, accessing the local community and social engagements or employment.  Additional senior responsibilities as a locality coordinator include writing rota's, deploying staff hours, attending service user reviews, supervising staff, and ensuring the services are operating in line with policies and procedures. There may be some travelling involved travelling to multiple sites.
