The North London Clinic provides a full care pathway from medium secure, low secure to a locked rehabilitation wing. The full multidisciplinary team works with men who have a diagnosis of mental disorder and associated challenging behavior.

Overall Objectives

To ensure that the department provides and maintains a high standard of record keeping, and a confidential and efficient service in accordance with the Referrals and Admissions Policy, contract Monitoring, Data Protection Act (1998) and Mental Health Act 1983.

 

Main Duties and Responsibilities 

  • To process, update, document and maintain information regarding patient Referrals and Admissions.
  • To attend Referrals meetings and prepare the agenda and take minutes.
  • Responsible for all referrals for The North London Clinic, Suttons Manor Hospital and Lily Close.
  • Assisting Hospital Director to ensure occupancy targets are met on a monthly/ yearly basis.
  • Accurately forecasting admissions/discharges from all 4 sites.
  • Responsible for ensuring all funding contracts are received for all patients admitted
  • Being the central contact for all referrals within the region
  • Sending admission alerts/ Patient lists to key MDT members and finance team at Head Office.
  • Adding referrals onto CAREnotes.
  • Chasing up referral decisions form funding panels, MoJ, Court, etc.
  • Maintaining spread sheet on referrals including reason for decline and time from referral to admission.
  • Responsible to for chasing assessment reports completed by MDT, checking for accuracy and quality prior to sending out to the referrer (Commissioner).
  • Ensuring assessments are arranged within 48 hours of a referral being received, with assessment reports/ care plan completed within 72 hours (where possible).
  • Responsible for data integrity of CAREnotes for the Hospital
  • To attend meetings and internal/ external statutory and mandatory training events including updates as and when required.
  • To maintain and develop effective information systems, data management processes and filling systems so as to meet statutory business requirements.
  • To take personal responsibility to ensure work practices and environment meet the requirements of ‘Health and Safety’ at work legislation and report any deficits to management.
  • To undertake any other task that commensurate with the posts level of responsibility.

 

 

Skills and Knowledge

  • The post holder will demonstrate the following knowledge and experience.
  • The successful applicant will need to hold a GCSE in English or equivalence. They also need experience/ understanding of referrals in a Mental Health setting.
  • Proficient in use of Microsoft Word, Excel, Outlook, Power Point, Excel.
  • Good organisational abilities, with good time management skills.
  • Excellent Telephone manner.
  • A flexible, collaborative attitude towards duties and responsibilities.
  • Excellent oral, listening and written skills demonstrating empathy and concern for others.
  • Ability to present information in an accurate and concise manner.
  • Recognise the effective utilisation of all appropriate development resources and opportunities.
  • Promote the ethos of continuous improvement disseminating any new knowledge gained to other colleagues.
  • Recognise and accept responsibility for own personal development
